DOT Physical Exam

STEP 1. Fill out a 1-page brief health history. Photo ID required.

STEP 2. Complete your DOT physical exam with doctor Kenn, or doctor Derek

– review of brief medical history

– height, weight, blood pressure, pulse rate, range of motion, breathing, eyes, ears

– discuss special medical conditions

– urinalysis to check for kidney function: blood, sugar, protein.

STEP 3. We complete your paperwork – Long Form MCSA-5875, Medical Card MCSA-5876.

  • You get the original DOT medical card, form MCSA-5876, to keep with you at all times you are driving.
  • You get a copy of the DOT medical examination report, form MCSA-5875.
  • Federal information requirements – We update the federal database with your new medical card information.
  • State information requirements – It is the driver’s responsibility to update their current medical card information with their state drivers license department.
    • *Utah CDL holders – We fax your medical card information to Utah DMV for you. No trips to DMV necessary!
      (Sorry we can’t do this for all other state CDLs – every state has different requirements.)

Payment options are cash, credit card, T-check, Com-check, or pre-authorized company account.

DOT Drug Alcohol Testing

Drug alcohol testing is not part of a DOT physical exam but your Company may request to have drug alcohol testing done at the same time. DOT drug testing and blood alcohol testing are separate tests and separate charges.

Certified Medical Examiners (FMCSA)

Effective May 21, 2014 – All interstate CMV drivers must get their medical certificates from a certified medical examiner listed on the Federal Motor Carrier Safety Administration’s National Registry of Certified Medical Examiners.
