What are the DOT physical requirements?
- The best way to understand the DOT physical requirements is to go through the checklist that the medical examiner goes through to conduct the physical exam. Here is a link to the FMCSA Medical Examination Report form.

Does a DOT physical include a drug test?
- Drug testing is not part of a DOT Physical exam but your Company may request to have drug alcohol testing done at the same time. DOT drug testing and breath alcohol testing are additional charges.
What drugs are tested for, if a DOT drug test is requested?
- The 5 Panel drug test analyzes urine for the following drugs:
- Marijuana (THC metabolite)
- Cocaine
- Amphetamines
- Opiates (including heroin)
- Phencyclidine (PCP)
What’s involved in a DOT physical?
- The process is pretty straight forward. We can complete your physical in 30 minutes. Here are our 3 easy steps to complete your DOT physical.
Why do I have to have a urine test?
- The urine test is just a kidney screen to test for sugar and protein. It’s done to determine that there is no early onset of conditions like high blood pressure breaking down the kidneys, or early signs of diabetes, or kidney infection.
Can I obtain a copy of my DOT physical form?
- Yes. We give you 2 copies of the long-form. One for the driver, one for the Company. If you need, we will fax the DOT form to your company. The examining doctor keeps a copy of the form for our office records.
Can the DOT medical card be laminated?
- Yes. We laminate your DOT medical card for you. Because you need to keep it with you at all times while you’re driving, lamination helps to reduce the wear and tear on the card.
How long is a DOT medical card good for?
- A DOT medical card is good for two years if you have no restrictions. A history of high blood pressure, taking high blood pressure medications, and/or taking oral medications for diabetes can restrict the medical card to one year.
How many 3-month DOT medical cards can you have?
- One only. You should have the condition, which caused the restriction, under control by the end of the 3-month window.
Is there a difference between a DOT medical and a DOT physical?
- No. These terms are used interchangeably. They are most often referred to as a DOT medical when referring to the DOT medical card, and DOT physical when referring to the DOT physical exam, and sometimes drivers refer to them as a CDL physical or CDL medical.
If I don’t have medical insurance can I still get a DOT physical?
- Yes. You do not need medical insurance to get a DOT Physical. Insurance is not a part of our DOT Physical exam service. Payment for the exam is your responsibility at time of service. Some insurance companies treat this exam as preventative health care and will not reimburse for it. If you have insurance and want to claim reimbursement we can give you a receipt to submit to your insurance company.
How much does a DOT physical cost without insurance?
- We’ve seen DOT physicals charged anywhere between $50 and $100. We charge the same price whether you have insurance or not. Payment for our DOT Physical service is $65.00 and payment options are cash, credit card, T-check, Com-check, or pre-authorized company account.
How do overweight truck drivers pass the DOT physical?
- Being overweight is not a restriction to passing the DOT Physical exam. It is the medical conditions that being overweight eventually leads to, such as high blood pressure, heart disease and kidney disease, that may affect your ability to safely drive a commercial vehicle. That is what the DOT Physical exam will determine – whether you have any of these restrictive medical conditions. Getting healthy and staying healthy is your best guarantee for your life and your livelihood.
What are the DOT physical blood pressure requirements?
Here are the medical guidelines according to the Federal Motor Carrier Safety Regulations (FMCSRs). Note that employers are allowed to impose more stringent medical requirements.
- A driver with a BP of less than 140 systolic and less than 90 diastolic may be medically certified to drive for a two-year period.
- A driver with a BP of 140 – 159 systolic and /or a BP of 90-99 diastolic, has stage 1 hypertension, and may be medically certified to drive for a one-year period. Certification examinations should be done annually thereafter and should be at or less than 140/90.
- A driver with a a BP of 160-179 systolic and/or a BP of 100-109 diastolic, has stage 2 hypertension, and is a candidate for antihypertensive drug therapy. The driver is given a one-time certification of three months to reduce his or her blood pressure to less than or equal to 140/90. Provided treatment is well tolerated and the driver demonstrates a BP value of 140/90 or less, he or she may be re-certified for one year from the date of the initial exam. The driver is certified annually thereafter.
- A driver with a BP at or greater than 180 systolic and / or 110 diastolic has stage 3 hypertension and is disqualified. The driver may not be qualified, even temporarily, until blood pressure is reduced to equal to or less than 140/90 and treatment is well tolerated. The driver may be certified for 6 months and biannually (every 6 months) thereafter if at recheck BP is equal to or less than 140/90.
Drivers with Type 1 or Type 2 diabetes, or kidney disease require treatment if their blood pressure rises above 130/80, since they already have a high risk of heart disease. See our article on “High Blood Pressure: Commercial Driver Risk?” for more information.
Can a driver with mild sleep apnea get a CDL?
- Obstructive sleep apnea does not necessarily disqualify you for a DOT medical card. Treatment with a CPAP machine and some basic lifestyle changes can help you get a restful sleep and maintain your commercial drivers license. What are the truck driving vision requirements?
- A commercial driver must have distant visual acuity of at least 20/40 in each eye. That means without corrective lenses, or with corrective lenses, you must be able to see 20/40 or better, with each eye individually as well as together. Your field of vision should be at least 70 degrees horizontally, and you must be able to recognize the standard traffic signal colors of red, green and amber.
What would cause me to fail a DOT physical?
- Over all the years we’ve been working with drivers, the biggest cause for concern we see is high blood pressure. Another concern is uncontrolled sugar in the urine. There are ways you can manage your blood pressure and blood sugar. See our tips to prepare for your DOT physical.
- Any condition that would cause a loss of ability to control, operate, or drive a commercial vehicle safely – loss or impairment of limbs, poor visual acuity, insulin-controlled diabetes, high blood pressure (more than 180/110), heart disease, respiratory disfunction, epilepsy, mental disorder, use of certain drugs, alcoholism. What if the DOT drug screen tests positive?
- If the urinalysis is positive for one or more of the drugs on the 5 panel drug test, then the driver will be contacted in person by the Medical Review Officer. The MRO will interview the driver to determine if there is an alternative medical explanation for the drugs found in the urine specimen. If the driver provides appropriate documentation, a confirmation test is performed for each identified drug using gas chromatography/mass spectrometry (GC/MS) analysis. GC/MS confirmation ensures that over-the-counter medications or preparations are not reported as positive results. If the MRO determines that it is a legitimate medical use of the prohibited drug, the drug test result is reported as negative to the employer. Otherwise the drug test will be reported as positive.
Do I need a medical card if I’m a local driver?
- We’ll answer this question as it pertains to the state of Utah, as we’re located in Salt Lake City. For other states please check with the motor carrier division of your state. For Utah the full documentation is available at the Motor Carrier Division Utah DOT.
- In summary, a driver qualifies under UDOT, and must carry a valid medical card, if driving a commercial vehicle that:
- Has a gross vehicle weight or gross combination weight of 10,001 pounds or more
- Is designed or used to transport more than 8 persons for compensation
- Is designed or used to transport more than 15 persons not for compensation
- Is used in transporting hazardous material

Generally you should not have any real problems, as long as the conditions do not limit your driving abilities. Just remember this is about making sure you and the public are safe.
The examining Dr. will need to assess the extent of the hernia to determine whether it is a restriction or not. He will also question you about your other conditions and your medication usage.

I don’t believe the CDL you are going for is as strict as the Class A drivers I deal with.
You may be dealing with a non-issue here. The medications that you are taking could effect your chances at a Class A CDL, but you are going for a lesser license, and you are going to be your own boss. So your drug testing and employment issues are all in your ball park. Tell the truth and you should not have any problems.

You H1 C test should not interfere with your CDL unless you are a insulin dependent diabetic, then you will be limited to intra-state driving or forced to get a waiver from the FMCSA to drive inter-state.

SP.GR 1.015, is a simple test measuring the concentration of water to waste ratio.
Normal ranges of Specific Gravity are from 1.002 to 1.030. For my drivers, it usually means that the drivers are, or are not, drinking enough water. Your urine should look like a pale ale, not an amber beer. Drink plenty of water – yes it makes you pee more often, but it’s a good thing.

/ November 6, 2011I have searched all over the internet and I am beside myself worrying about this. I am 42 years old and I have just been perscribed vyvance a couple of months ago due to the fact I had trouble concentrating. The medicine is like a miracle drug for me. I take 30mg in the morning and 30 mg at night. I also take 200mg of NEFAZODONE every night i go to bed which I have been on for 6 years for my ocd. I do not drink any alchohal whatsoever and do not abuse any other narcotics. I am really not worried about those as much as the fact that I am perscribed 15mg of oxycodone and it says to take 1 every six hours but I usually do not do that during the day. I take one in the morning when I wake up and usually when I get home I take another one. I am only perscribed 100 a month and I am taking them as directed. I live in Florida and own a car lot and taking my CDL license and it requires a physical and a drug test. I am worried about telling them about these drugs allthough they do not make me drowsy at all. I always tell the truth and I am really fighting over this one. I can save 7 to 10 thousand a month transporting my own cars as opposed to paying someone. I will be driving a dually truck and it is not an 18 wheeler. It is short trips and I am only hauling 5 cars at a time. Do I put all this on the paperwork and show them my perscriptions before I take the test or do I get one of those drinks at the healthfood store to pass the test and not say anything. I am really having trouble with this. If I thought in any way this would impair my driving and hurt anyone I would not even began to do it but I do not feel any effects that make me sleepy or that would impair my ability to perform this task. I was just taught honesty is always the best policy. Could you please advise me on how to handle this so I can get my cdl license. Thank you so much for your time in this. Please advise!

If it is the company policy, then yes they can request these tests.
Being out on a medical leave and taking prescribed medications can throw off one of the two tests requested, but the other test will support your negative findings.
So unless you have some other issue with the drug test request, I’d go for it if the job is worth it.

Every drug test for the DOT requires you to have a split sample urine test. The first sample and the split are sent to the lab for testing. If, for some reason, your first sample comes back positive, the medical review officer [MRO] will call you. He wants to know what medications, prescriptions or odd foods you may have eaten or taken within the past few days. Then he makes a determination as to whether you pass or fail. Some schools are doing instant testing with no splits. That is not a qualified test protocol. So ask how the testing was completed, why didn’t the MRO call to confirm or deny your test results. Or was it an instant, non-DOT approved test procedure?

The results from the drug test are given to the company requesting the test. These results, to the best of my knowledge, do not travel with you. Unless there is some new ruling with CSA 2010 point reporting, but I don’t think so.

DOT requires you have a minimum of 20/40 vision in both eyes, either non-corrective or with corrective lenses. Your peripheral vision is within normal limits.
All that being said, you could be driving skills tested with a FMCSA exemption. Check with the FMCSA for the requirements for that particular exemption.

More than likely you will be OK. When the company does its drug testing you will have a positive finding. When the medical review officer for the drug testing company contacts you about what meds you take, just tell him what you take and how often, this should make a positive test negative. That will work via the drug testing, but the company may have another in-house policy. Make sure the company is aware of your present conditions and what medications you take beforehand so that they have a heads-up.

/ August 30, 2011I have just been hired as a schwan deliver driver, and am a 90% disabled veteran with chronic Reiters syndrome, so I take 25mg percocet and 15mg ambien daily/nightly for sleep and pain, will this DQ me from obtaining this job. Your DOT examiner needs to see your blood sugar levels below 120. That is the level for blood sugar spillover into the urine, his concern is not whether you have ever had a blood sugar blackout, it’s – Could you have a blood sugar blackout while driving a bus full of little people?

The DOT examiner can extend your DOT card to one year once you pass the 140 / 90 BP. Since you are taking blood pressure medication your DOT card will only be good for one year. So go back to your DOT examiner and if your pressure is below 140 / 90 he should extend it, but it will be from the date of the examination. If you let the first one expire he will most likly give you a on year card. Also the company you applied for the job with should understand that your correction of the high blood pressure was something you were unaware of and with correction you really do have a one year card. I am a diabetic, non-insulin dependant, when I took the test my blood sugar was 350, but in the past few days I have gotten it down to about 170. What are the requirements for blood sugar limits?? I have never had a low or high blood sugar blackout- will I have to get an exemption?

/ August 13, 2011I recently applied for a job driving. The comapny requires a 1 year DOT card and nothing less. When I went for my physical I came up with a 178/113 blood pressure
reading. I went to my doctor and he perscibed medicine for me. I did not know that I had high blood pressure. I went back to the place that did the physical and passed they gave me a 6 month card. That does not qualify me for the job. My blood pressure is fine now but I understand that if I go back I can only get a 6 month card. If I let the DOT card expire and then reapply for the job and take the physical and come in with a 140/90 or less will I be able to get a one year card? Had I known that I had high blood pressure I could have gone to my doctor first then gone to the physical and passed and gotten a one year card.

WOW!! Talk about a DOT physical with a happy ending…
I have been doing physical examinations for the thirty years I have been a doc. This is not any test that I know of. A basic hernia examination is a finger into the inguinal canal and cough. Testicles in hand and jump up and down????????? – No hernia examination I’ve ever heard of.
So unless the company has some very special request for some weird exam, I would ask to have a real doctor finish the examination and I would request a male doctor if at all possible.

/ July 29, 2011I passed every part of the DOT exam yesterday. I have no health problems at all. I am not overweight and I have no pains in my back or stomach. The last thing the nurse told me to do was “drop my pants to my ankles so she could cup my testicules while I jump up and down”. I asked her what for…she said to check for a hernia. I told her that she just did an initial check for any signs of a hernia while I was laying on the table but she insisted that I must drop my pants. Now…just to be clear…I have no problem getting naked. I told her I would be happy to walk through her lobby naked if she wanted me to. But, as an American who is trying to get a $10 an hour job which will net me about $25 a week more than what unemployment currently pays me…I think the whole “let me cup your balls and do jumping jacks” is a bit much. I was amazed that I was the one viewed as the weird one and not the insanity of the law. So, I told her to get bent and left. Of course I was not given the certificate or the card. I told my prospective employer that I thought it was bizarre and outrageous that I was required to do such a thing in order to be allowed to drive a small box van around town. Amazingly he told me that he also has to have his balls cupped and do jumping jacks every two years. He literally told me that I should have man’d up and suffered the 10 seconds of indignity so I would have been given the card. This completely misses the point??? I live in Ft Walton Beach Florida. I am dumbfounded that this country has descended into such depths of injustice and authoritarian rule that in order for me to get a poverty level job, I must be sexually exploited. I have a 21 year old daughter and the way this is headed…I honestly can see the day when she will not be able to get a job unless she performs oral sex on the CEO. What the hell is going on in this country? HELP! I need this job but I feel a responsibility to my daughter and future generations to STOP the unconstitutional abuses by our government.

It could, when the DOT examiner does his examination, one of the things he is looking for is things that would be unsafe for you as a driver and how it effects your ability to maintain control of a commercial motor vehicle. Part of that examination is to test for virtigo. If he feels the condition is unsafe then he will not pass you, if he feels there is something that can help you maintain, he may make a suggestion for you to follow-up with a specialist. All you will need to do is get hearing aids and the examiner will indicate you need to wear your hearing aids to pass the DOT examination.
This does not place a time restriction or limitation on your DOT certificate and you won’t need an exemption.

The doctor said it’s not large enough to have surgery at present, but to keep an eye on it for one more year. Once this is repaired and replaced, and I finish recovery process, would I be able to return to normal duties as a truck driver under your DOT regulations provided that my medical physican gives me the “ok”?

The thing that will be limiting to your DOT medical card will be the high blood pressure and taking medication to maintain it. This will limit your Medical Card to one year. Also, if the company you will be driving for has concerns about your sleep apnea they may request an updated study, but is up to the company. The blood thinners are not a problem. I’m a 55 year old male who is considering trucking for a second career. I have high blood pressure that is controlled with medication, mild sleep apnea 19 events over 8 hours and I take blood thinners for a blood clot. Will taking blood thinners or a combination of all prevent me from obtaining my medical card and driving a tractor trailer? Thanks for your time.

/ July 2, 2011It is me again…I am a cerfified dot drug screen collector. I was certified as a dot drug screen trainer. I know my collection certification is good for five years.. I was told at the time I recieved my training my training certificate was good for seven. I have been told now that new dot regulations require that a trainer of dot collections must be certified every two years…I have not been able to comfirm this. Could you help me with this and if so tell me where I can find this in dot codes. Thanks so much.

/ July 2, 2011Thanks for the help on DOT records….We have a MRO and regular Doctors. If a regular doctor does a DOT pysical with no required Drug Screen…but the same man needs to have a 10 panel non-dot drug screen (revieved by MRO) to get in to a plant for employment can that drug screen (just the MRO signed chain) be stored on his personel health record ? I would think not…This is an on going issue. So since a school bus driver is a required dot drug screen the signed MRO chain (not the individual drug results my be stored on personal health record unless . ask not to . If the drug screen is not preformed at pysicians office that is doing the dot pysical should the drug screen results be stored with the dot pysical if the client brings in a copy of his signed MRO chain . Normally with HIPFA compliance, the last employer is not at liberity to state the reason for firing.
But that does not mean it will not follow him, especially if he works around nuclear power and refused to comply with a drug test.
