What are the DOT physical requirements?
- The best way to understand the DOT physical requirements is to go through the checklist that the medical examiner goes through to conduct the physical exam. Here is a link to the FMCSA Medical Examination Report form.

Does a DOT physical include a drug test?
- Drug testing is not part of a DOT Physical exam but your Company may request to have drug alcohol testing done at the same time. DOT drug testing and breath alcohol testing are additional charges.
What drugs are tested for, if a DOT drug test is requested?
- The 5 Panel drug test analyzes urine for the following drugs:
- Marijuana (THC metabolite)
- Cocaine
- Amphetamines
- Opiates (including heroin)
- Phencyclidine (PCP)
What’s involved in a DOT physical?
- The process is pretty straight forward. We can complete your physical in 30 minutes. Here are our 3 easy steps to complete your DOT physical.
Why do I have to have a urine test?
- The urine test is just a kidney screen to test for sugar and protein. It’s done to determine that there is no early onset of conditions like high blood pressure breaking down the kidneys, or early signs of diabetes, or kidney infection.
Can I obtain a copy of my DOT physical form?
- Yes. We give you 2 copies of the long-form. One for the driver, one for the Company. If you need, we will fax the DOT form to your company. The examining doctor keeps a copy of the form for our office records.
Can the DOT medical card be laminated?
- Yes. We laminate your DOT medical card for you. Because you need to keep it with you at all times while you’re driving, lamination helps to reduce the wear and tear on the card.
How long is a DOT medical card good for?
- A DOT medical card is good for two years if you have no restrictions. A history of high blood pressure, taking high blood pressure medications, and/or taking oral medications for diabetes can restrict the medical card to one year.
How many 3-month DOT medical cards can you have?
- One only. You should have the condition, which caused the restriction, under control by the end of the 3-month window.
Is there a difference between a DOT medical and a DOT physical?
- No. These terms are used interchangeably. They are most often referred to as a DOT medical when referring to the DOT medical card, and DOT physical when referring to the DOT physical exam, and sometimes drivers refer to them as a CDL physical or CDL medical.
If I don’t have medical insurance can I still get a DOT physical?
- Yes. You do not need medical insurance to get a DOT Physical. Insurance is not a part of our DOT Physical exam service. Payment for the exam is your responsibility at time of service. Some insurance companies treat this exam as preventative health care and will not reimburse for it. If you have insurance and want to claim reimbursement we can give you a receipt to submit to your insurance company.
How much does a DOT physical cost without insurance?
- We’ve seen DOT physicals charged anywhere between $50 and $100. We charge the same price whether you have insurance or not. Payment for our DOT Physical service is $65.00 and payment options are cash, credit card, T-check, Com-check, or pre-authorized company account.
How do overweight truck drivers pass the DOT physical?
- Being overweight is not a restriction to passing the DOT Physical exam. It is the medical conditions that being overweight eventually leads to, such as high blood pressure, heart disease and kidney disease, that may affect your ability to safely drive a commercial vehicle. That is what the DOT Physical exam will determine – whether you have any of these restrictive medical conditions. Getting healthy and staying healthy is your best guarantee for your life and your livelihood.
What are the DOT physical blood pressure requirements?
Here are the medical guidelines according to the Federal Motor Carrier Safety Regulations (FMCSRs). Note that employers are allowed to impose more stringent medical requirements.
- A driver with a BP of less than 140 systolic and less than 90 diastolic may be medically certified to drive for a two-year period.
- A driver with a BP of 140 – 159 systolic and /or a BP of 90-99 diastolic, has stage 1 hypertension, and may be medically certified to drive for a one-year period. Certification examinations should be done annually thereafter and should be at or less than 140/90.
- A driver with a a BP of 160-179 systolic and/or a BP of 100-109 diastolic, has stage 2 hypertension, and is a candidate for antihypertensive drug therapy. The driver is given a one-time certification of three months to reduce his or her blood pressure to less than or equal to 140/90. Provided treatment is well tolerated and the driver demonstrates a BP value of 140/90 or less, he or she may be re-certified for one year from the date of the initial exam. The driver is certified annually thereafter.
- A driver with a BP at or greater than 180 systolic and / or 110 diastolic has stage 3 hypertension and is disqualified. The driver may not be qualified, even temporarily, until blood pressure is reduced to equal to or less than 140/90 and treatment is well tolerated. The driver may be certified for 6 months and biannually (every 6 months) thereafter if at recheck BP is equal to or less than 140/90.
Drivers with Type 1 or Type 2 diabetes, or kidney disease require treatment if their blood pressure rises above 130/80, since they already have a high risk of heart disease. See our article on “High Blood Pressure: Commercial Driver Risk?” for more information.
Can a driver with mild sleep apnea get a CDL?
- Obstructive sleep apnea does not necessarily disqualify you for a DOT medical card. Treatment with a CPAP machine and some basic lifestyle changes can help you get a restful sleep and maintain your commercial drivers license. What are the truck driving vision requirements?
- A commercial driver must have distant visual acuity of at least 20/40 in each eye. That means without corrective lenses, or with corrective lenses, you must be able to see 20/40 or better, with each eye individually as well as together. Your field of vision should be at least 70 degrees horizontally, and you must be able to recognize the standard traffic signal colors of red, green and amber.
What would cause me to fail a DOT physical?
- Over all the years we’ve been working with drivers, the biggest cause for concern we see is high blood pressure. Another concern is uncontrolled sugar in the urine. There are ways you can manage your blood pressure and blood sugar. See our tips to prepare for your DOT physical.
- Any condition that would cause a loss of ability to control, operate, or drive a commercial vehicle safely – loss or impairment of limbs, poor visual acuity, insulin-controlled diabetes, high blood pressure (more than 180/110), heart disease, respiratory disfunction, epilepsy, mental disorder, use of certain drugs, alcoholism. What if the DOT drug screen tests positive?
- If the urinalysis is positive for one or more of the drugs on the 5 panel drug test, then the driver will be contacted in person by the Medical Review Officer. The MRO will interview the driver to determine if there is an alternative medical explanation for the drugs found in the urine specimen. If the driver provides appropriate documentation, a confirmation test is performed for each identified drug using gas chromatography/mass spectrometry (GC/MS) analysis. GC/MS confirmation ensures that over-the-counter medications or preparations are not reported as positive results. If the MRO determines that it is a legitimate medical use of the prohibited drug, the drug test result is reported as negative to the employer. Otherwise the drug test will be reported as positive.
Do I need a medical card if I’m a local driver?
- We’ll answer this question as it pertains to the state of Utah, as we’re located in Salt Lake City. For other states please check with the motor carrier division of your state. For Utah the full documentation is available at the Motor Carrier Division Utah DOT.
- In summary, a driver qualifies under UDOT, and must carry a valid medical card, if driving a commercial vehicle that:
- Has a gross vehicle weight or gross combination weight of 10,001 pounds or more
- Is designed or used to transport more than 8 persons for compensation
- Is designed or used to transport more than 15 persons not for compensation
- Is used in transporting hazardous material

Although I remain active in sports, the presence of this tissue causes a little discomfort from time to time, but in no way renders me unable to perform any tasks. This type of injury will require documentation.
If you are going to drive only within your state borders, it will be easier. Just check with your DMV to see what the states rules and regulations are.
To pursue an interstate medical card will require an exemption that you can only get from DOT and FMCSA. Check out the requirements from either of these websites to determine if this is something you want to pursue. It is possible, but a bit tough to do. Lots of hoops, but do-able.

Sleep apnea is a serious condition, if left untreated. When treated successfully with a C-pap, it does not effect the drivers medical.
In most cases the company may have some standard, but it usually does not mean termination from a job. I would not be overly concerned about it until it becomes an issue. I would ask the company what their policy is on this type of issue first.

I am making an assumption you are asking about doing the urine testing during the DOT exam. That test is to determine the integrity of the kidneys. During that phase of the examination you are not observed, unless requested by the company.
During a DOT drug test you could be observed during the exam. This should be done only by request, and the observer is to be of the same sex as you, no opposite sex observation.

The blood thinner should not cause any problems.
With the blood clot in the lung, you want to make sure you bring a note from you pulmonary specialist stating you are good to go. Your DOT examiner will need a copy for your file when he does the exam.

There is not a given law that states you can not be a driver, but, studies have demonstrated the long term effect of the stroke is a likelihood of having another one. If your neurologist can clear you and you bring that note with you during the DOT physical, then the examining doctor can clear you for a period of up to one year, if he feels that you would be safe for yourself and for the public at large.

The other effects of the medication could be a disqualifier from driving.
I would bring a note from you physician stating how long and how often you use this medication. A bus-load of kids could be a bit stressful and may trigger your need to medicate, now we go into the other effects of the medication. It would make me wonder about your safety and the safety of the children.

I have been taking them off and on since 1996 and they do not make affect my ability to drive or hold a job. As I said before I take it on a PRN basis.

/ January 27, 2012I had a Stroke a few years ago i was out on medical leave and came back to work for the past 2 years i had to go for my Medical card physical as my company requested. I passed the physical and they said i had to have a stress test so i took the stress test and they said i passed that but the physician giving me the physical and stress test said he cannot pass me because i had a stroke? I have been cleared by the specialist doctors that saw me during and after my storke and say i can drive! I live in Ohio and employeed in West Virginia. I have been out of work for 2 weeks now and I need to get back to work before i loose everything! Any suggestions please help!! Is there a law or anything saying if you have a stroke you can no longer drive???

You are kidding, right?
No enema needed for a DOT examination.
It’s a basic physical examination: Height, weight, visual acuity, blood pressure, etc.
You will need to give a urine sample for the examination, so come with a need to pee. The urine sample is to check for protein, blood and sugar in the urine and to rule-out any other kidney disfunction.

The metformin is to control your blood sugar as a borderline diabetic.
It will not show up in the DOT drug test. It will limit your DOT medical certificate to one year at a time. That’s normal and routine.

Your blood pressure needs to be below 140 / 90, even with the medication, just to clear for a one year certificate.
The Gabpentin has these warnings: This drug may make you dizzy or drowsy or cause blurred vision. Do not drive, use machinery, or do any activity that requires alertness or clear vision until you are sure you can perform such activities safely. Limit alcoholic beverages.
This will have any DOT examiner looking very closely at your ability to operate a commercial motor vehicle safely, until you can demonstrate otherwise.

Also, currently investigating a chronic joint/muscular pain and fatigue situation that’s been getting worse last 5 years (I’m 30 yr old female); just recently on Gabpentin (600mg BID). I’m seeking schooling for CDL for the first time and so far I’m having to give up just about everything in order to do it…will I even be able to “pass” a DOT physical on these meds????

/ January 20, 2012I have been looking and looking for the recommendations for the sleep study. My Hubby was flagged for the sleep study with a 33 BMI. No other questions asked at all. His company says REGULATION( scary huh that this is their director of safety talking) REQUIRES drives who have a 35 BMI AND 17 inch neck to be tested within 6 months. Yet their own DR claims RECOMMENDATION is 33 BMI no need to ask any questions and now their Medical Adviser claims RECOMMENDATION is 29 BMI. I have been researching for days and I do KNOW its not regulation anywhere yet. ( Maybe I should apply for head of safety at this GLOBAL company hubby works for) But what are the recommendations that DRS are supposed to be following right now? Are there even any in regards to JUST BMI and neck size? Or are they as screwed up as they appear?

So far, insulin dependent drivers are limited to intra-state driving only.
OTR drivers can be orally medicated and with proper control of their diabetes can get a one year medical certificate.
The DOT does have a few, very stable insulin dependent drivers testing on insulin waivers, but you would have to check with the Federal DOT on this issue.

Not real sure how to answer this question.
If you do not have a valid medical certificate, you cannot drive a commercial motor vehicle.
Your driving privileges for a four wheeler are not effected, unless that is your state law. (You’ll need to check).
There are other ways to fail the DOT exam that can be reversed, like high blood pressure, sleep apnea, drug uses. etc. But if you don’t want to drive now, then don’t.

As a diabetic and a OTR driver, your choice is limited. The DOT says no drivers on insulin, period.
But if you want to jump through lots of DOT hoops, you might try getting a diabetic waiver from them.
As a driver, traveling intra-state, you can be insulin dependent / insulin pump, but not a interstate driver.
I have seen one or two drivers that have received the waiver from DOT, but it was a real pain in the butt, but do-able.

Well, each trucking company has it’s own set of rules.
DOT will want you to go through a drug rehab program, usually a basic drug awareness type program. Then they will test you fairly routinely for a while to make sure you got it. After that you should be able to go back to driving.

Yes, you can get your medical certification.
It will be limited to one year and you will probably need to bring your most recent stress test with you – make sure it is within 2 years since the last stress test.
Also, with the blood thinners, make sure that they won’t interfere with things like tiredness or drowsiness, these will be things your examiner will have questions about. He has to protect you and the public.

The medications that you are taking at this time are for day-time sleepiness and joint pain.
Any examiner is going to ask why do you suffer with these conditions: Do you have sleep apnea? What level of arthritis do you have?
These will be questions that will need to be answered before you have to worry about the drugs themselves.
The examiner is looking out for your and the public’s safety. Sleep apnea is an issue that must be looked at very closely and the need for medications for joint pain, Gabapentin, will be questioned because it is also used for seizure control.
Just be prepared to provide information to the examiner as to the uses and needs for these medication along with proper documentation from your primary healthcare provider.

Once you have a valid stress test done within any two year time frame, your medical examiner should now move your medical certificate out for the remainder of the one year cert. I would begin to wonder what else the examiner is looking for?
There is no reason you cannot have your personal physician do your DOT examination, as long as he understands the rules and regulations surrounding the issuance of the DOT medical certificate. Three stress test in the past month is more than enough for any DOT examiner, as long as the results are within normal limits.

/ December 7, 2011If the DOT physician has certified me for 3 months under the condition that I have additional studies done during that time frame, can I go to another physician to get my DOT renewed at the end of the 3 months without complying with the original doctors request? My DOT Doc ordered me to have a stress test due to my having had stent put in 10 years ago and not having had a stress test in over 2 years – long story short..stress done, found a block, stent put in and now back to work – but card is only good for 3 months then he wants me to have ANOTHER stress test to make sure everything is okay before he will renew my card for the rest of year. Can I just go to my family physician when the card expires? Is there any reason he can’t certify me for the rest of the year…I mean really! I have had 3 stress tests now in the last month, surgery and a nuclear stress test after that and my cardiologist released me to go to work…just getting the feeling this guy just doesn’t want to certify me.

This will depend on the company policy. Most companies will allow your doctor to do the examination, just make sure they fill the forms out correctly. But your company may have a policy that makes in manditory for you to use their DOT examiner. Just check with them.

According to the DOT he will need to have a stress test every two years.
As for the two year physical, the guidelines recommend a yearly physical so we know where he is regarding his ability to be safe on public highways. It will also depend if he is taking any type of blood pressure medications.

My brother had a heart attack 3 years ago. Stents were put in place. He has passed 3 of the stress tests for his heart. He has been going yearly for his CMV phyiscal since his heart attack. Last one was done in Oct this year. His heart doctor told him he felt there was no need to do the stress test because everything looked great on his numbers. Is it a requirement to have a stress test yearly as well? How long after a heart attack can a person go back to every 2 years on physicals?

It looks like the examiner is trying to determine if you may be color blind.
But the primary reason for the color test is to determine your ability to distinguish red, amber/yellow and green.
Not really sure as to why they would do this. Especially since you have been driving for so long.
If it was me I would probably go somewhere else and get another exam that just checks for the primary colors, not a color blindness examination. Thanks for the questions. Trucker Doc.

/ November 19, 2011I have a cdl-a driver for 16 years. Last week for my Dot physical i failed my color vision portion of the exam. I can , and have always been able to see the colors red, green , and amber. No problem when the Dot examer shows me colored blocks, colored strips. This examer only had the book of each page showing a circle of numerour dots on a page and ask me to tell her what the numers were on each page. I only could see, and answer on about half of the test pages. I explained to the examier that I can see the colors as long as they are not in her dot book. Whats going on with this?

Well, I don’t believe it will show up on a DOT drug test.
But that being said, why don’t you just take care of the high blood pressure for good and not worry about it at all? There many ways to lower blood pressure without taking medications. You might want to explore some of those answers and resolve this issue once and for all. There really isn’t a quick fix for the condition but there are many viable answers to correct the condition for good.

As I understand it, in the state of Utah for sure, Yes, the driver must have a medical certificate to operate a vehicle being used for commerce. So the truck/trailer and driver must be DOT approved, DOT numbers on the truck and trailer and medical card for the driver.

It does not matter whether he knows or not. On the DOT medical examination form that you fill out, it will ask if you are a diabetic and also how you control the condition. So you are the one who is going to tell him about the insulin.
If you want to continue driving you will need to get a waiver from DOT. Your examining Dr. should ask quite a few questions regarding your insulin usage to help him determine which direction he should point you in.
